HTML5 Forms

Hey everyone, I am pretty new to this and I have an assignment for school on forms. I have to create a form to collect first and last name, email and other information like that. I have to include sections underneath the form that will format and display the entered data after its submitted. I am having trouble, truthfully, I am clueless on how to do this. I have been searching the internet but I am coming up empty. Any help would be really appreciated.
